What this actually is

A second, separate discount on top of the premium tax credit that cuts your deductible, your copays and coinsurance, and your annual out-of-pocket maximum. A plan with a $750 deductible might have a $300 one for you; a $5,000 out-of-pocket maximum might be $3,000. Here is the thing almost nobody is told, and it is the whole reason this entry exists: cost-sharing reductions apply ONLY if you buy a SILVER plan. Every year people who qualify buy the cheaper Bronze plan to save $40 a month, and silently forfeit thousands of dollars of deductible help — a subsidised Silver plan is frequently cheaper in total than the Bronze plan next to it. Check your Eligibility Determination Notice: if it says you can choose a plan with lower copayments, coinsurance and deductibles, followed by (04), (05) or (06), you qualify, and you must pick Silver to get it. The lower your income within the range, the larger the discount.

Who qualifies, and what trips people up

You must qualify based on income and enrol in a Silver-category Marketplace plan. Bronze, Gold and Platinum plans get the premium tax credit but not these savings. Catastrophic plans get neither. American Indians and Alaska Natives have special, more generous cost-sharing rules — ask specifically. If you have a Silver plan and your income changes so you lose cost-sharing reductions, that opens a Special Enrollment Period to switch plans. You only learn the exact savings after you apply and shop Silver plans, so apply first and compare second.
